Glaucoma is a leading cause of vision loss, often developing without noticeable symptoms until permanent damage has occurred. Advances in eye care now allow glaucoma to be treated earlier and more safely with Minimally Invasive Glaucoma Surgery (MIGS).
At Eye Associates of South Texas, our ophthalmologists offer advanced glaucoma treatments, including MIGS procedures designed to lower eye pressure while minimizing surgical risk.
What Is MIGS?
Minimally Invasive Glaucoma Surgery (MIGS) refers to a group of procedures that reduce intraocular pressure (IOP) using microscopic instruments and tiny incisions. These procedures improve the eye’s natural drainage system so fluid can exit the eye more efficiently.
Compared to traditional glaucoma surgery, MIGS procedures typically provide:
- Smaller incisions
- Faster recovery
- Lower risk of complications
- Reduced dependence on glaucoma medications
Although MIGS may not lower eye pressure as dramatically as conventional surgery, it offers an excellent option for patients with mild to moderate glaucoma.
How MIGS Works
MIGS procedures enhance fluid drainage from the eye by improving or bypassing the eye’s natural drainage pathways.
Many MIGS procedures are performed during cataract surgery, which already helps reduce eye pressure. When combined, cataract surgery and MIGS can further decrease the need for glaucoma medications. The MIGS portion of the procedure typically adds only a few minutes to cataract surgery.
Types of MIGS Procedures
MIGS techniques vary depending on how they improve drainage.
Micro-Stent Procedures
Tiny devices are placed in the eye to help fluid drain more effectively.
Examples include:
- XEN Gel Stent
- iStent Inject
- Hydrus Microstent
Angle-Based Procedures
These procedures modify the eye’s drainage structures—such as the trabecular meshwork and Schlemm’s canal—to improve fluid outflow.
Common techniques include:
- Trabectome
- Kahook Dual Blade
- OMNI Surgical System
In some cases, multiple MIGS techniques may be combined to maximize pressure reduction.
MIGS vs. Traditional Glaucoma Surgery
Traditional glaucoma surgeries such as trabeculectomy can significantly lower eye pressure but often involve longer recovery and a higher risk of complications.
MIGS procedures offer:
- A better safety profile
- Quicker recovery
- Effective pressure reduction for early to moderate glaucoma
This makes MIGS an attractive treatment option for many patients.
Who Is a Candidate for MIGS?
MIGS may be recommended for patients who:
- Have mild to moderate primary open-angle glaucoma
- Experience side effects from glaucoma medications
- Still have elevated eye pressure after other treatments
- Are already planning cataract surgery
An ophthalmologist can determine whether MIGS is the most appropriate treatment based on your eye health and glaucoma severity.
